Hydroclean Advance Wound Dressing

HydroClean Advance is a sterile hydro-responsive wound dressing pre-moistened with Ringer’s solution, designed for the moist treatment and debridement of acute, chronic and hard-to-heal wounds.

The dressing continuously releases Ringer’s solution into the wound while simultaneously absorbing wound exudate. Its superabsorbent polyacrylate core traps wound fluid and helps remove substances that can interfere with healing, supporting wound cleansing, autolytic debridement and the development of healthy granulation tissue.

HydroClean Advance is particularly suitable during the cleansing and granulation phases of wound healing.

How HydroClean Advance works:

HydroClean Advance uses an interactive rinsing and absorption mechanism.

The dressing contains a superabsorbent polyacrylate core embedded in cellulose fibres and pre-activated with Ringer’s solution.

Once applied to the wound:

1. Ringer’s solution is released into the wound bed, helping soften and loosen devitalised tissue.
2. Wound exudate is simultaneously absorbed back into the dressing.
3. Bacteria, wound debris and other substances contained within the exudate are drawn into the absorbent core.
4. The superabsorbent polymer helps retain the absorbed fluid.
5. Excess matrix metalloproteinases that may interfere with wound healing are bound within the dressing.
6. This continuous rinsing-and-absorption process supports wound cleansing and preparation of the wound bed for granulation and further healing.

The dressing can continue providing moisture to the wound for up to 3 days.

Dressing construction:

HydroClean Advance consists of:

• A superabsorbent polyacrylate core
• Cellulose fibres
• Ringer’s solution
• A polypropylene knitted wound-contact layer
• Silicone strips on the wound-contact surface to reduce adherence
• A water-impermeable polypropylene film on the outer surface
• Hydrophobic polypropylene non-woven backing

The outer layer helps prevent premature drying of the dressing while allowing moisture to remain available to the wound.

Suitable wound types include:

HydroClean Advance may be used for a wide variety of acute and chronic wounds requiring moist wound management and debridement, including:

• Venous leg ulcers
• Arterial leg ulcers, following appropriate vascular assessment
• Diabetic foot ulcers
• Pressure ulcers / pressure injuries
• Chronic wounds
• Poorly healing wounds
• Acute wounds
• Surgical wounds
• Traumatic wounds
• Abrasions
• Burns, subject to appropriate clinical assessment
• Donor sites
• Malignant or fungating wounds where clinically appropriate

It is particularly suited to wounds during the:

• Cleansing / debridement phase
• Granulation phase

HydroClean Advance is suitable for:

• Dry wounds
• Non-exuding wounds
• Lightly exuding wounds
• Moderately exuding wounds

It is not intended for heavily exuding wounds.

How to use:

HydroClean Advance should be used by a healthcare professional or by a person who has received appropriate training from a healthcare professional.

1. Assess and prepare the wound according to the prescribed wound-care protocol.
2. Check the dressing pouch for damage before opening.
3. Use an aseptic technique.
4. Remove the dressing carefully without squeezing out the Ringer’s solution.
5. Place the white side directly against the wound.
6. The side with the blue HydroClean printing should face upwards.
7. Ensure the dressing is in direct contact with the wound bed.
8. The dressing should overlap the wound margins.
9. Secure HydroClean Advance using an appropriate dressing-retention material.
10. Apply a suitable secondary dressing or fixation system according to the wound and exudate level.

For low or non-exuding wounds, an appropriate film dressing may be used for fixation.

For wounds with greater exudate, an appropriate absorbent secondary dressing may be required.

HydroClean Advance may also be used underneath appropriate:

• Compression bandages
• Compression hosiery

where compression therapy has been clinically prescribed.

Important: Do not cut HydroClean Advance.

The dressing contains a sealed superabsorbent core and must not be cut to size or otherwise mechanically damaged.

Select the appropriate dressing size for the wound.

If HydroClean Advance becomes damaged, superabsorbent particles or fibres may enter the wound. If this occurs, the wound should be thoroughly irrigated with sterile saline or Ringer’s solution.

Deep or cavity wounds:

Standard HydroClean Advance should not be used to pack deep wounds.

For deep or cavity wounds, use the specifically designed:

HydroClean Advance Cavity

The cavity dressing can be placed into the wound and covered with standard HydroClean Advance where clinically appropriate.

A separate HydroClean Advance Mini 3cm Round dressing is also available for very small wounds and appropriate small cavity wounds.

Dressing changes:

HydroClean Advance may remain in place for:

Up to 3 days

depending on:

• Wound condition
• Exudate level
• Progress of debridement
• Condition of the dressing
• Clinical judgement

Change the dressing sooner if:

• Its absorption capacity has been reached
• The dressing becomes saturated
• The dressing dries out prematurely
• Leakage occurs
• The wound needs reassessment
• The dressing becomes damaged or displaced

More frequent dressing changes may be appropriate during intensive wound cleansing.

Removal:

In most cases HydroClean Advance can be removed without causing additional pain.

If the dressing has adhered to the wound:

1. Moisten it with sterile physiological saline or Ringer’s solution.
2. Allow the dressing to loosen.
3. Remove gently to avoid trauma to the wound bed.

Important information:

HydroClean Advance should only be used as part of an appropriate wound-management plan.

The underlying cause of impaired wound healing must also be assessed and treated.

For example:

• Venous disease may require compression therapy
• Diabetic foot ulcers may require pressure off-loading
• Arterial wounds require vascular assessment
• Pressure injuries require pressure redistribution
• Infection may require specific antimicrobial or systemic treatment

Do not use HydroClean Advance:

• In patients with a known hypersensitivity or intolerance to any component
• For heavily exuding wounds
• On dry necrotic foot wounds caused by arterial occlusive disease
• As a substitute for appropriate surgical treatment of third-degree burns

Infected wounds:

Infected wounds and wounds with a high bacterial burden should only be treated with HydroClean Advance under the supervision of an appropriately qualified healthcare professional.

This is particularly important where an occlusive film dressing is being used for fixation.

HydroClean Advance does not replace appropriate treatment of wound infection.

Topical medicines and disinfectants:

Do not routinely apply topical medicines or disinfectants underneath HydroClean Advance.

The dressing may absorb these substances and potentially reduce their effectiveness.

Where antimicrobial or other topical treatment is required, compatibility should be assessed by the treating healthcare professional.

Bleeding risk:

Use particular caution in patients at increased risk of bleeding, including patients taking:

• Anticoagulants
• Other medicines that increase bleeding risk

At dressing changes, remove HydroClean Advance carefully.

If necessary, moisten the dressing thoroughly with sterile saline or Ringer’s solution before removal.

Sensitive periwound skin:

Where the surrounding skin is fragile, damaged or particularly sensitive, an appropriate skin-protection product may be used around the wound.

Regularly monitor for:

• Maceration
• Irritation
• Redness
• Skin breakdown

Use on the sole of the foot:

If HydroClean Advance is applied to the plantar surface of the foot, the patient should not stand or walk directly on the dressing.

Pressure could damage or rupture the dressing and release particles from the absorbent core.

Appropriate pressure off-loading should be used.

Pregnancy and children:

The manufacturer states that the safety and effectiveness of HydroClean Advance in pregnant women and children have not been established.

Use in these groups should therefore be based on appropriate clinical assessment.

Do not:

• Reuse
• Reprocess
• Re-sterilise
• Use if the pouch is damaged
• Use beyond the stated expiry date

Reuse can compromise the integrity and performance of the dressing and increase the risk of irritation, delayed healing, pain or infection.
