How medical-grade Manuka honey works

Medical-grade Manuka honey provides several useful effects within the wound environment.

Its high osmotic activity draws fluid towards the wound surface, which can help:

• Hydrate devitalised tissue
• Soften slough and necrotic tissue
• Support autolytic debridement
• Encourage movement of wound debris into the dressing
• Maintain an appropriately moist wound environment

Manuka honey also provides antibacterial activity and can help reduce unpleasant wound odour associated with bacterial burden.

An initial increase in wound exudate may occur because of the osmotic action of honey. A suitably absorbent secondary dressing may therefore be required, particularly during the early stages of treatment.

Activon Tulle

Activon Tulle is a soft, conformable knitted viscose mesh dressing impregnated with 100% medical-grade Manuka honey.

The non-adherent mesh keeps the honey in direct contact with the wound while allowing wound exudate to pass through into an appropriate secondary dressing.

It is particularly useful for superficial wounds where direct and even contact with medical-grade honey is required.

Suitable wound types include:

• Superficial wounds
• Partial-thickness wounds
• Full-thickness wounds
• Cuts and abrasions
• Pressure ulcers / pressure injuries
• Leg ulcers
• Surgical wounds
• Burns
• Infected wounds
• Graft sites
• Donor sites

Activon Tulle may also assist with wound debridement when used in conjunction with Activon Tube.

How to apply Activon Tulle

1. Cleanse the wound according to the appropriate wound-care protocol.
2. Gently dry the surrounding skin.
3. Remove the clear protective film from both sides of the dressing.
4. Apply Activon Tulle directly to the wound bed.
5. Either side of the dressing may be placed against the wound.
6. The dressing may be cut to size where necessary.
7. Cover with an appropriate secondary dressing selected according to the wound and exudate level.
8. Secure according to clinical requirements.

Dressing changes – Activon Tulle

Depending on:

• Wound exudate
• Oedema
• Wound condition
• Secondary dressing
• Clinical assessment

Activon Tulle may remain in place for up to 7 days.

The honey becomes progressively diluted by wound exudate and therefore less effective.

Change the dressing when:

• The colour changes significantly
• The honey has been substantially diluted
• The secondary dressing becomes saturated
• Leakage develops
• The wound requires reassessment

If necessary, irrigate the wound with saline to facilitate dressing removal.

Activon Tube

Activon Tube contains 100% medical-grade Manuka honey without a carrier dressing.

It can be applied directly to the wound bed and is particularly useful for:

• Sloughy wounds
• Necrotic tissue requiring autolytic debridement
• Pressure ulcers
• Surgical wounds
• Burns
• Graft sites
• Malodorous wounds
• Appropriate cavity wounds

It can also be used to top up another honey dressing where wound exudate has diluted or washed away some of the honey.

How to apply Activon Tube

1. Cleanse the wound appropriately.
2. Gently dry the surrounding skin.
3. Remove the sterile tube from its packaging.
4. Twist off the cap.
5. Apply the honey liberally and directly to the:
   • Wound surface, or
   • Appropriate wound cavity.
6. Cover with a suitable secondary dressing.
7. Select sufficient absorbency to manage the expected wound exudate.

The product is intended for single-patient use.

Once opened, the current Activon Tube may be used for up to 90 days, provided it is handled and stored correctly.

Secondary dressing

Both Activon Tulle and Activon Tube require a suitable secondary dressing.

Because honey can initially increase wound exudate, a highly absorbent or superabsorbent dressing may sometimes be required.

The secondary dressing should be selected according to:

• Wound exudate
• Wound depth
• Wound location
• Condition of the surrounding skin

Possible discomfort

Medical honey can cause a temporary:

• Stinging sensation
• Burning sensation
• Increase in wound discomfort

This is associated with the osmotic action of honey and may subside after application.

If significant pain persists:

• Discontinue use
• Irrigate the wound with saline
• Seek appropriate clinical advice

Advancis notes that if Activon Tulle causes discomfort, Algivon may sometimes be better tolerated because it releases honey more slowly.

Diabetes

Activon honey products contain glucose.

Advancis recommends monitoring patients with diabetes appropriately when these products are used. Diabetic wounds also require appropriate assessment of:

• Infection
• Circulation
• Neuropathy
• Pressure and off-loading

Contraindications

Do not use Activon products in patients with a known allergy or hypersensitivity to:

• Honey
• Bee venom
• Bee-derived products

Stop treatment and seek appropriate advice if a significant hypersensitivity reaction develops.

Infection

Activon Manuka honey provides antibacterial activity and may be used as part of the management of appropriately assessed infected wounds.

However, it does not replace appropriate treatment of serious or spreading infection.

Seek clinical assessment if there is:

• Increasing redness
• Swelling
• Increasing pain
• Purulent discharge
• Spreading inflammation
• Fever
• Rapid wound deterioration

Systemic antibiotic treatment or other intervention may be required where clinically indicated.

Important information

Activon products should be used as part of an appropriate wound-management programme.

The underlying cause of chronic wounds must also be addressed, for example:

• Compression therapy for suitable venous leg ulcers
• Off-loading for diabetic foot ulcers
• Pressure redistribution for pressure injuries
• Vascular assessment for arterial wounds
• Appropriate antimicrobial treatment for clinical infection

Do not:

• Reuse dressings
• Share an opened tube between patients
• Use damaged sterile products
• Use beyond the stated expiry date
