What is Clear Advantage Spirit used for?

Clear Advantage Spirit is intended for the management of male urinary incontinence.

The sheath is worn externally over the penis and connected to an appropriate:

  • Urinary leg bag

  • Bed / night drainage bag

  • Other compatible urine collection system

Urine passes through the sheath outlet and drainage tubing into the connected collection bag.

Unlike an indwelling urinary catheter, the sheath:

  • Does not enter the urethra

  • Does not enter the bladder

  • Is worn externally

It may therefore provide a suitable urine-management option for appropriately assessed men who experience urinary leakage but do not require an indwelling catheter.

Self-adhesive silicone sheath

Clear Advantage Spirit is manufactured as a self-adhesive silicone sheath.

The internal adhesive region helps secure the sheath to the penis without requiring a separate external adhesive strip.

Silicone provides a soft, flexible sheath material designed to conform to the body while allowing the user or carer to monitor the underlying skin.

Hydrocolloid adhesive

The Spirit range uses a hydrocolloid adhesive system.

The adhesive is positioned differently according to the selected style.

Choosing the appropriate adhesive position and sheath length is important for:

  • Secure fixation

  • Comfort

  • Reducing leakage

  • Reducing unwanted sheath movement

  • Avoiding excessive pressure on the skin

Style 1 – Standard

Style 1 has a standard sheath length with the adhesive positioned centrally.

The manufacturer describes it as suitable primarily for men with a good penile length.

Style 1 may be appropriate where:

  • There is sufficient penile length for a standard sheath

  • A conventional adhesive position provides adequate security

  • Extra-wide adhesive coverage is not required

Style 2 – Pop-On

Style 2 is a shorter sheath, with the adhesive positioned further towards the tip.

This design can be particularly useful where the penis is:

  • Short

  • Retracted at rest

  • Difficult to accommodate within a conventional full-length sheath

The shorter configuration reduces the amount of sheath material that needs to be rolled along the penile shaft.

Style 3 – Wideband

Style 3 is a standard-length sheath with additional adhesive coverage.

The increased adhesive area is designed to provide a more secure attachment.

It may be particularly useful for men who:

  • Are physically active

  • Experience sheath movement

  • Have difficulty maintaining adhesion with a standard sheath

  • Require greater adhesive contact

Choosing the correct diameter

Correct sheath sizing is particularly important.

The available Clear Advantage Spirit diameters are:

  • 24mm

  • 28mm

  • 32mm

  • 36mm

  • 40mm

The diameter refers to the penile sheath size, not the drainage outlet or tubing diameter.

An appropriate manufacturer measuring guide should be used to determine the correct size.

A sheath that is too large may:

  • Slip

  • Leak

  • Detach prematurely

A sheath that is too tight may:

  • Cause discomfort

  • Restrict circulation

  • Cause pressure or skin damage

If uncertain, seek advice from a continence nurse or other appropriately trained healthcare professional.

Choosing the correct style

Selecting the right diameter is only one part of fitting.

The appropriate style should also be chosen.

Choose Style 1 – Standard where:

  • There is good penile length

  • Standard adhesive coverage is adequate

Choose Style 2 – Pop-On where:

  • Penile length is reduced

  • The penis tends to retract at rest

  • A shorter sheath is easier to apply

Choose Style 3 – Wideband where:

  • There is adequate penile length

  • Additional fixation is required

  • The user is particularly active

  • Standard sheaths repeatedly loosen or detach

Connection to a drainage bag

The sheath outlet is designed to connect to an appropriate urinary drainage system.

This may include:

  • Daytime leg bag

  • Night drainage bag

The connection should be secure without pulling on the sheath.

Ensure:

  • Tubing is not kinked

  • The drainage bag is positioned appropriately

  • The drainage bag has its own support

  • The weight of a filling bag does not pull directly on the sheath

A leg bag should normally be supported with appropriate straps or a sleeve.

Applying the sheath

Always follow the manufacturer's fitting instructions.

In general:

  1. Wash and dry your hands.

  2. Ensure the penis and surrounding skin are clean and completely dry.

  3. Check the skin for redness, soreness, broken areas or irritation.

  4. Measure and select the correct sheath diameter.

  5. Choose the appropriate Style 1, 2 or 3 configuration.

  6. Position the sheath according to the manufacturer's instructions.

  7. Roll or apply it smoothly without twists or folds.

  8. Ensure the adhesive area makes appropriate contact with the skin.

  9. Connect the sheath to the appropriate drainage bag.

  10. Check that the drainage tubing is unrestricted.

Do not apply the sheath over significantly damaged or inflamed skin unless specifically advised by an appropriate healthcare professional.

Avoid excessive tension

Once connected to the drainage bag, ensure that:

  • The tubing has sufficient slack

  • Normal movement does not pull on the sheath

  • The drainage bag is adequately supported

  • Clothing does not trap or kink the tubing

Excessive pulling can result in:

  • Discomfort

  • Sheath displacement

  • Leakage

  • Premature detachment

Changing the sheath

Male urinary sheaths are generally intended for single daily use, with UK continence formularies commonly specifying replacement approximately every 24 hours.

Follow the manufacturer's instructions and individual continence-care plan.

Replace sooner if:

  • The sheath becomes loose

  • Leakage develops

  • The sheath becomes damaged

  • Skin irritation occurs

  • Adhesion is no longer adequate

Do not reuse a used sheath.

Skin care

Regular inspection of the underlying skin is important.

Check for:

  • Redness

  • Soreness

  • Swelling

  • Pressure marks

  • Broken skin

  • Moisture damage

  • Allergic or irritant reactions

If persistent skin problems occur, discontinue use and seek advice from a continence nurse or healthcare professional.
